Does a dental implant hurt? An honest answer from the dentist

Quick answer: DOES IT HURT?

NO. Placing a dental implant under local anaesthesia is not painful. Patients report: vibrations while the implant site is prepared, a sensation of pressure and the sound of the instruments. After the procedure, the pain is “minimal” and manageable with standard analgesics.

Why do patients expect pain?

  • Childhood memories of procedures from the 80s/90s
  • Forum discussions and rumours
  • Visual anxiety about the procedure
  • The prevalence of dental phobia

How the anaesthesia works

  1. Surface anaesthesia (applying a gel)
  2. Local anaesthesia by injection (effective in 2–3 minutes, lasts 1.5–4 hours)
  3. Optional sedation for anxious patients

Pain during the procedure

Patients feel vibrations, a sensation of pressure, the sound of the instruments and the water cooling — but not pain when adequately anaesthetised.

After the procedure — the first 24 hours

  • Mild facial swelling (peaking on day 2–3)
  • A sensation of pressure
  • Mild pain, manageable with paracetamol or ibuprofen
  • Minor bleeding is normal

After the procedure — the first 7 days

Days 1–3: mild swelling and pressure; days 4–7: the swelling subsides; day 7–10: suture removal.

Special patient groups

  • Anxious patients: oral or IV sedation recommended
  • Older patients (65+): suitable after a health assessment
  • Children/adolescents: not before age 18 (rare exceptions at 16)
